Responsible to: Director of Life Enrichment

Scope of Responsibility: The Memory Care Program Coordinator (MCPC) is responsible for the development and execution of the interdisciplinary programming schedule for the Memory Care neighborhoods, including recreational and social activities, according to state/federal regulations, memory care professional standards and the policies and procedures of Golden Years Homestead (GYH) Campus.

Essential Functions:

  1. Assist in administering the life enrichment programs that meet the psychological and activity needs of residents of the memory care neighborhoods, including creating diverse opportunities for meaningful pursuits, daily pleasures, life enrichment, exercise, friendships, social interaction, growth and learning, fun and enjoyment, personal routines and neighborhood rituals.
  2. Prepare the monthly life enrichment calendar for residents of the memory care neighborhoods, including programs designed for residents with dementia or memory care needs.
  3. Conduct most of the scheduled events included in the life enrichment calendar and coordinate the execution of events by other departments (nursing).
  4. Provide assist memory care residents in transport to life enrichment events off the memory care neighborhoods scheduled for the main resident population but will return to memory care neighborhoods to continue overseeing programming for residents who remain on the memory care neighborhood.
  5. Complete resident assessments and chart documentation as needed, including assessments (e.g. life history, MDS), activity notes/attendance, and care planning related to recreational needs.
  6. Perform delegated functions as directed by Director of Life Enrichment and monitor the quality of said functions through continual quality improvement.
  7. Assure resident’s rights are met, including the right to refuse or individualized treatment, and monitor the community response to resident concerns through continuous quality improvement.
  8. Flexible hours to accommodate special programming, evenings/weekends and willingness to work in all communities as needed.
  9. Make routine/scheduled visits to residents who may not participate in group activities as directed; assist room-bound residents by visiting with them, writing letter, running errands, and other activities as appropriate.

Position Requirements:
Education & Experience

  1. Certification in recreational programming, certification in dementia care, or other specialized Alzheimer’s or dementia care or programming training
  2. Experience in LTC or working with elderly.
  3. Must have special interest in working with people who have memory challenges
  4. Demonstrate creativity in memory care programming that keeps scheduled experiences enjoyable, meaningful, and dynamic (avoids stagnancy)
  5. High School Diploma or equivalent desirable but not required.
  6. Basic computer skill required.
  7. Must have a valid driver’s license in good standing.

  1. Ability to work with a wide variety of personalities in a pleasant and professional manner.
  2. Special interest in older adults and the aging population.
  3. Ability to maintain enthusiasm and energy needed to keep residents engaged and stimulated.
  4. Professional image to enhance positive public relations.

  6. Ability to work independently and as a team without constant supervisory directive.

Golden Years is seeking someone who enjoys driving, but also loves interacting with people. We need a Transportation Driver to safely drive our residents back and forth to doctor's appointments and bring new residents to our facility from local hospitals. CDL is not necessary for this position.

Schedule:

  • 40 hours per week, Monday – Friday 8:00A - 4:30P
  • Voluntary opportunities for overtime in the morning or on weekends

The primary responsibility is providing transportation to the residents of Golden Years. This includes:

  • Providing mobility assistance to residents to get from their room to the exit of the facility
  • Enthusiastically engaging with residents, providing excellent customer service and socialization.
  • Physically assisting with boarding and departing the vehicle
  • Driving the company owned vehicle safely, obeying all laws and safety guidelines.
  • Ensuring safe arrival inside the desired destination
  • Prompt pickup of resident at requested time whenever possible

When transportation needs do not fill an entire workday, this position assists with other maintenance tasks such as:

  • Assist housekeeping with floor care and/or trash removal.
  • Performing unscheduled maintenance tasks as directed (will train on basic maintenance tasks)

Job Requirements / Qualifications:

Education & Experience
1. High school graduate or equivalent.
2. Experience serving geriatric population preferred.

Gina Martin

Employee Benevolence Fund

Gina Martin believes in paying it forward. She received help when she needed it, and now she helps others whenever she can. Gina began working for Greencroft Goshen five years ago. During her tenure, she experienced some personal problems of her own, including financial hardships. Her manager helped her apply for a gift from the Employee Benevolence Fund. Each campus has its own fund, managed by the Greencroft Communities Foundation, and the process to apply is straightforward. To be eligible, an employee must have worked at their campus at least one year. They must be in good standing with no disciplinary actions or excessive absences. “I was so grateful,” Gina said. “It helped me climb out of a hole.”
