The Life Enrichment/Memory Care Program Director will be responsible for organizing, implementing, evaluating, and directing the overall operation of engaging and innovative Life Enrichment Programs for the Healthcare campus. The director is also responsible for direct supervision of the Memory Care Support Household programming.

Job Requirements/Qualifications:

  • Associates or Bachelor's degree from an accredited education institution in therapeutic, recreation, occupational therapy, gerontology, or other healthcare related field required.
  • Rec Therapist preferred.
  • Minimum 3 years in a supervisory capacity in a healthcare setting, preferably serving seniors.
  • Minimum 2 years experience working with dementia or Alzheimer's residents in the last 5 years.
  • Must complete the required dementia training within a regulatory timeframe.

Job Description:

Responsible for the daily coordination of activities, problem-solving activity schedules, supply needs, and assist in the development of activities that meet the social, mental, physical, and spiritual needs of the residents. In addition, serve as the team leader to assure team members are informed of what is happening on a day-to-day basis and provide support to team members in facilitating the daily activities.

Job Description:

Home Care Attendants will maintain a pleasant and safe environment, supervises client activities, assists with activities of daily living, provides emotional support and communicates changes in the client’s status to his/her supervisor. Services will be performed in client homes on the Greencroft campus and throughout the community at large.

Job Requirements/Qualifications:

  • Must be 18 yrs old or older
  • Must be able to read, write, and follow instructions
  • High School diploma or GED, recommended
  • Certified Home Health Aide or CNA, preferred

Scope of Responsibility: Coordinates team members with outside contractors and vendors, working closely with Director of Facilities and Plant Operations in responsibilities related to the department.

Job Description:

  • Inspects apartments upon vacancy, during turnover process, and completion of turnover to ensure is progressing timely and the apartment is ready to be rented.
  • Assists in setting carpet dates and completion dates.
  • Works with Renovation Team to set priorities and make sure special requests are handled appropriately.
  • Works with contractors to make sure work is being completed timely and to Greencroft standards.
  • Remodel apartments including demolition, framing, drywall hanging, finish work, and trim carpentry.
  • Orders specialty items needed to ensure turnovers are completed timely.
  • Basic plumbing and electrical work as assigned including running new circuits, wiring residential fixtures, and plumbing in toilets, sinks, and showers in either plastic or copper pipe.

Education & Experience

  • High school diploma or GED required.
  • Three or more years of general carpentry experience preferred
  • Experience in construction and physical plant operations and ability to work well with others.
  • General carpentry, dry-walling, electrical, and plumbing skills to complete projects.
  • Physical ability to climb ladders, stoop, carry construction supplies, and work in an efficient and safe manner.
  • Must possess a current driver’s license.
  • Ability to work without close supervision while maintaining Greencroft Maintenance Department standards of conduct and job performance.

Job Description:

Responsible for promoting self determination and quality of life in older adults. Responsible for networking services, documentation and providing family support for Healthcare residents.

Duties include:

  • Interview resident and family upon admission to finalize all admission paperwork. Assess resident/family needs for appropriate interventions.
  • Discuss advance directives, support choices made by the resident and keep an annual update of medical directives of all residents.
  • Develop and maintain plan of care with established problems, goals and approaches on each resident.
  • Establish and update discharge plans on all residents and coordinate services prior to discharge
  • Facilitate behavior team meeting and document acting plans for behavior management.

Job Requirements/Qualifications:

  • Bachelor’s degree in Social Work, Sociology, Psychology or related field, or equivalent experience required.
  • Previous experience working with older adults. Nursing home experience preferred.
  • Familiarity with Medicare, Medicaid, VA and Pre-admission screening programs, or willingness to learn quickly

Schedule:

  • Monday - Friday
  • Weekend and holiday rotation

Gina Martin

Employee Benevolence Fund

Gina Martin believes in paying it forward. She received help when she needed it, and now she helps others whenever she can. Gina began working for Greencroft five years ago. During her tenure, she experienced some personal problems of her own, including financial hardships. Her manager helped her apply for a gift from the Greencroft Goshen Employee Benevolence Fund. Each campus has its own fund, managed by the Greencroft Communities Foundation, and the process to apply is straightforward. To be eligible, an employee must have worked at Greencroft at least one year. They must be in good standing with no disciplinary actions or excessive absences. “I was so grateful,” Gina said. “It helped me climb out of a hole.”
